Please complete the following steps to complete the process:
- Complete the Financial Institution Notification Form and send it to your account representative. Request that your broker transfer stock to the PVSF Charitable Gift Account #1148-0336.
- Complete the Schwab Contribute to a Charitable Gift Account (CCGA) Form Sections 1 and 6. At the head of the form, contribute TO: Charitable Gift Account Number should be PVSF Charitable Gift Account #1148-0336.
- When the CCGA form is complete, please send to:Schwab Fund for Charitable Giving
