Thank you for Gifting Physical Stock
Please complete the following steps to complete the process:

1. Complete the Financial Institution Notification Form and send it to your account representative. Request that your broker transfer stock to the PVSF Charitable Gift Account #1148-0336.
2. Complete the Schwab Contribute to a Charitable Gift Account (CCGA) Form Sections 3, 5, and 6. Note that you must have your signature notarized in Section 5. At the head of the form, contribute TO: Charitable Gift Account Number should be PVSF Charitable Gift Account #1148-0336.
3. If you are sending security certificates, we recommend you send them to Schwab with the completed form via registered, certified or overnight mail service and request a return receipt.
4. When the CCGA form is complete, send the signed form and stock to:
