To Send a Physical Stock Certificate
Proceed as follows:
- Complete the PVSF
Notification Form and send it to the address given on the form.
- Complete the Financial
Institution Notification Form and send it to your account representative.
Request that your broker transfer stock to the PVSF Charitable Gift
Account #1148-0336 at Schwab.
- Complete the Schwab Contribute
to a Charitable Gift Account (CCGA) Form Sections 3, 5, and 6. Note
that you must have your signature notarized in Section 5. At the head
of the form, contribute TO: Charitable Gift Account Number should be
PVSF Charitable Gift Account #1148-0336.
- If you are sending security certificates, we
recommend you send them to Schwab with the completed form via registered,
certified, or overnight mail service and request a return receipt.
- When the CCGA form is complete, if you send
by US mail, send the signed form and stock to:
