To Transfer Stock from a Non-Schwab Account
Proceed as follows:
- Complete the PVSF Notification Form and send it to the address given on the form.
- Complete the Financial
Institution Notification Form and send it to your account representative.
Request that your broker transfer stock to the PVSF Charitable Gift
Account #1148-0336 at Schwab.
- Complete the Schwab Contribute to a Charitable Gift Account (CCGA) Form Sections 4 and 6. Note
that, contrary to the directions on the form, you do not need
to complete Section 5 of the form; requesting that Schwab initiate the
stock transfer from another institution will lengthen the process considerably.
At the head of the form, and in Section 4, transfer TO: Charitable Gift
Account Number should be PVSF Charitable Gift Account #1148-0336.
- When the CCGA form is complete, if you send
by US mail, send to:
